Equity Project Enhanced Case Manager (Westside Regional Center)

Pay Rate$3,388-$3,737 Monthly (Step 1-5)

(*Rate may be higher with direct employment experience supporting our population)

To effectively provide enhanced case management services to adults and children who are currently receiving no POS funded services, with the goal of increasing service understanding and utilization as well as improving quality of life indicators for individuals and families. 

REQUIREMENTS:

  • Proven exceptional case management and client advocacy skills (minimum of 3 years experience) that includes creative and alternative approaches to addressing client needs
  • Bilingual Spanish/English
  • Ability to effectively manage a reduced caseload while providing enhanced case management services to include increased face to face and telephone contact
  • Expert working knowledge of Regional Center and generic (e.g. SSI, IHSS, CCS, EPSDT, and mental health,) resources
  • Exceptional assessment skills in identifying possible areas of client/family needs within the IPP format
  • Skills and experience in addressing specifically the needs of clients who are ethnic minorities and typically underutilize Regional Center services
  • Exceptional abilities in building rapport and trust with clients and families
  • Have consistently met documentation requirements on current caseload
  • Have a past performance in collaborative efforts within WRC (FRC, Community and Clinical Services) and outside WRC (community agencies, schools and other community stakeholders)
  • Have outstanding communications skills
